Create Intake Form

Creates an empty intake form. The name must not be empty. requireCompletionBeforeCreate makes the form's required questions a condition of creating an event. Add questions with Events_AddIntakeQuestion, and attach the form to an event type with the Organization API's EventTypes_SetIntakeForm. Needs an organization admin token. Returns the new form id.
